Provides follow-through of the case management program to ensure members receive appropriate healthcare and will triage new onset medical problems for the physician to prevent readmissions. Care coordination of post hospitalized patients will provide for the coordination of medically necessary services to support the primary care physicians in the delivery of healthcare. Gathers and documents patients' data in an electronic system, and communicates data to RN, NP, or MDs using technologies such as the Internet, computers, telephones, digital assessment tools, and telemonitoring equipment. Job Responsibilities: Coordinates follow-up care appointments and/or assists in directing care to physician clinic nurse or ITC clinic and/or medical services for post-hospitalized patients and assures timely and complete communication to ITC clinics when applicable. Assesses patients' clinical information and effectiveness of current medical services through phone or in-person interviews at the hospital at the time of discharge. Investigates and solves problems relating to patient responses indicating continued medical problems, plan of care failures, knowledge deficits, and other responses of concern. Provides health services counseling regarding identified patient knowledge deficits, such as dietary information, medication orders explanation, and rationale for immediate follow-up with primary care physician/ITC, etc. Identifies need for non-covered services and provides options for the patient with the assistance of hospital case manager(s) and/or social worker(s). Identifies urgent issues and forwards them to the case manager, case manager supervisor, case management manager, or PCP as indicated. Comply with all HIPAA guidelines, maintaining the confidentiality of clients and patients. Schedule post-discharge appointments. Comply with Federal and respective state laws regulating health management organizations and telephone information centers and all department standards and policies and procedures Licenses/Certifications. Perform duties ONLY within the scope of practice approved by the state in which the patient is located. Provides utilization management and quality improvement in accordance with utilization policies and processes. Ensures adherence to the Quality Management Program. Performs other job-related duties as assigned. Desired Qualifications and Requirements: Bachelor's / College Degree Registered Nures Minimum of 2 years' experience of Case Management Experience Excellent communication, time management and computer skills Excellent oral & written communication and listening comprehension. Customer Service Skills for interacting the doctors, physicians, and patients Proficiency with Microsoft Applications Highly organized with a strong attention to detail Comfortable in a fast-paced environment Approachable, professional, discrete, and personable Ability to provide high-quality customer service and follow through on all assignments.
